The following is a brief summary of Intellect Solutions (Intellect) employee benefits program. Benefits may vary slightly depending on employment status and geographic location of individuals. Please consult our Human Resources Department for additional information.

MEDICAL, DENTAL AND VISION INSURANCE
Intellect offers the health insurance programs for REGULAR FULL-TIME employees (as determined by the carrier of the policies). Eligible employees and dependents are eligible for comprehensive medical, dental, and vision insurance coverage through United Health Care. Employee costs are deducted on a pre-tax basis per pay period. Intellect pays up to 50% of the employee’s insurance cost and provides a provision to add a spouse and/or dependent at favorable rates. Coverage begins immediately after all applications have been submitted successfully.

VACATION/ SICK LEAVE
Paid vacation is available to regular full-time and regular part-time employees. Employees can use accrued vacation with Intellect and it is provided based on the following calculations:

  • During the first 3 (three) years of employment, vacation time will be earned at the rate of .0384 hours per hour worked.
  • During the 4th (fourth) and following years of employment, vacation time will be earned at the rate of .0586 hours per hour worked. Employees can carry over up to 40 hours of accrued vacation annually.

401(k) QUALIFIED RETIREMENT PLAN
Intellect provides eligible employees with a 401(k) Qualified Retirement Plan where our contribution currently begins at 90 days of full-time employment – which is an excellent means of long-term savings for your retirement. Traditional Safe Harbour 401(k) Plan was the elected tiered basic plan:

  • Matching contribution equals 100% on the first 3% of participant’s compensation which is deferred as an Elective Deferral.
  • Matching contribution equals 50% on the next 2% of participant’s compensation which is deferred as an Elective Deferral.

The Finance Department can provide a copy of the Summary Plan Description which contains the details of the plan including eligibility and benefit provisions based upon the employee’s annual salary. In the event of any conflict in the description of any plan, the official plan documents, which are available for your review, shall govern. If you have any questions regarding this plan, see the plan administrator. Intellect retirement contributions may be changed prospectively, at any time, at employee’s annual salary’s sole discretion.

EDUCATIONAL TUITION ASSISTANCE PROGRAM
To encourage professional development, Intellect offers tuition assistance for master degree level programs, not to exceed $500 per credit hour, up to $1,500 per course. This program is available for up to one (1) course per semester with a maximum of three (3) courses per year. Degree and/or courses must be related to an employee’s occupation. Each course must be preapproved by the employee’s supervisor and Intellect senior management. Classes started prior to Intellect Solutions employment do not qualify. Assistance is issued following the successful completion of coursework and submission of grade received:

  • Grade A- and above = 100% of allowable assistance
  • Grade B- to B+ = 75% of allowable assistance
  • Grade C and below = 0% of allowable assistance

If an employee leaves Intellect within two (2) years following the successful completion of the degree or last course (whichever is applicable), he or she will be responsible for reimbursing Intellect for 100% of the total assistance amount. Full payment will be deducted from the employee’s final pay check consistent with the wage deduction authorization form that employees are required to execute as part of their application for the Educational Tuition Assistance Program. To participate in this program, you must be a full-time employee, and in good standing with individual performance and project performance goals. Employees in the probation period (90 days of employment at Intellect) do not qualify for assistance. Dropped or incomplete programs for reasons other than a serious medical condition require repayment of assistance provided by Intellect for courses taken within the previous two (2) years. Employees are prohibited from performing course related work during office hours or with the use of company resources.

EMPLOYEE REFERRAL PROGRAM
Intellect Solutions employee referrals are an effective way for recruiting top-notch employees. If you refer a person who then is hired and remains in the job for at least three months, you may receive a minimum reward of $1,000 and maximum reward up to $3,000, less applicable taxes.

DISABILITY BENEFITS
As part of your benefits, Intellect provides Short-Term and Long-Term disability coverage at no cost to employees. You may be eligible to collect disability benefits as a covered insured individual during any leave of absence for medical reasons. LIFE AND ACCIDENTAL DEATH AND DISMEMBERMENT (AD&D)
As part of your benefits, Intellect provides Life and Accidental Death and Dismemberment (ADD) coverage. For more information about Life and ADD benefits please contact the Human Resources Department. You will receive up to 25% discounted rate for Life Insurance with Liberty Mutual. An agent is available to all participating employees.

HOLIDAYS
Intellect observes the following paid holidays per year:

  • New Year’s Day

  • Washington’s Birthday (President’s Day)
  • Memorial Day
  • Independence Day
  • Labor Day
  • Columbus Day
  • Veterans Day Observed
  • Thanksgiving Day

JURY DUTY/MILITARY LEAVE
Employees will be granted time off to serve on a jury or military leave without pay. However, all regular employees either full-time or part-time will be kept on the active payroll until their civic duties have been completed. A copy of the jury duty summons and all other associated paperwork are required for the personnel file.

PERFORMANCE- BASED INCENTIVE PLAN
Employees may receive bonuses from time to time. These are based on individual merit, the company’s profitability, and any other factor deemed significant by the company. Any bonus granted is solely within the discretion of Intellect senior management.

PROFESSIONAL ORGANIZATION MEMBERSHIP
Monetary assistance may be available for any professional memberships directly related to your employment at Intellect.

TRAINING
Intellect offers paid training to enable employees to hone their skills. Training must be job related and is subject to management approval. All training must be preapproved by the employee’s supervisor and Intellect senior management. To participate in this program, you must be a full-time employee and in good standing with individual performance and project performance goals. Employees still within their probationary period (first 90 days of employment at Intellect) do not qualify for training reimbursement without an explicit written approval from Intellect executive management. If an employee leaves Intellect within one (1) year following the successful completion of the training, he or she will be responsible for reimbursing Intellect for 100% of the reimbursed costs. Full payment will be deducted from the employee’s final pay check. If an employee leaves Intellect within two (2) year following the successful completion of the training, he or she will be responsible for reimbursing Intellect for 50% of the reimbursed costs. Full payment will be deducted from the employee’s final pay check

SOCIAL SECURITY/ MEDICARE
Intellect withholds income tax from all employees’ earnings and participates in FICA (Social Security) and Medicare withholding and matching programs as required by law.

HOME AND AUTO INSURANCE
Intellect employees will receive up to 25% discounted rate for Home and Auto Insurance with Liberty Mutual. An agent is available to all participating employees.

PAYROLL DEDUCTIONS
You may elect pre-tax deductions from your pay check for qualify following benefits plans:

  • DepCare FSA
  • Flexible Spending Accounts
  • Insurance Election
  • HSA Plans, please contact the Benefits Specialist of Human Resources Department to review those options.

WORKERS’ COMPENSATION
On-the-job injuries are covered by our Workers’ Compensation insurance policy. This insurance is provided at no cost to you. If you are injured on the job, report the incident immediately to your supervisor. Consistent with applicable state law, failure to report an injury within a reasonable period of time could jeopardize your claim.

COBRA BENEFITS
The Federal Consolidated Budget Reconciliation Act (COBRA) gives employees and their qualified beneficiaries the opportunity to continue health insurance coverage under the Intellect health plan when a “qualifying event” would normally result in the loss of eligibility. Some common qualifying events are resignation, termination of employment, or death of an employee; a reduction in an employee’s hours or leave of absence, divorce or legal separation and a dependent child no longer meeting eligibility requirements. Under COBRA, the employee or beneficiary pays the full cost of coverage at Intellect group rates plus an administration fee
